The applicant has provided the Aadhar card as the address proof in the applicant which contains the following address.
Surekapuram Colony, Jabalpur Road, Bathua, Aghauli, Mirzapur, PIN Code-231001, Mobile number-7379105911,
The following application was made by the applicant for the renewal of driving licence which filled the particulars are as follows.

Public information officer in the office of regional transport officer Mirzapur must provide the following information point wise to the information seeker as follows.
1- In the first page of the PDF document both old and new driving licence are available for taking perusal.

Public information officer must provide the reason for sending current driving licence at the address of Village-Nibi Gaharwar, Rampur Ghat Marg, Dist-Mirzapur, Uttar Pradesh.

4- Provide the name and designation of the staff under whom monitoring processing of the renewal of driving licences takes place.

Last time my renewed driving licence was sent at the address of Surekapuram Colony, Jabalpur Road, Mirzapur city PIN Code-231001,but this time it has been sent to village and post- Nibi Gaharwar, PIN Code-231303.
Regional transport officer Mirzapur must tell the reason for not providing the renewed driving licence on my current address Surekapuram Colony, Jabalpur Road District Mirzapur, PIN Code-231001.
The matter concerns the forest rule in the working of the regional transport officer Mirzapur whose staff are working arbitrarily showing incompetence of the staff for the post.
With this grievance four documents are attached as follows
1- A scan copy of the driving licence which was to be renewed.
2- Aadhaar card of the applicant as address proof of the applicant.
3- Receipt of the fee payment Rs.400 to the Public Authority regional Transport Office Mirzapur.
4- Copy of the application for the renewal of the driving licence of the applicant containing both current and permanent address of the applicant in five pages. Consequently there are a total of eight pages.

Most respected sir the address written on my driving licence renewed by the regional transport officer mirzapur as follows. Village and post office- Nibi Gaharwar, Rampur Ghat marg District Mirzapur. During the earlier renewal the applicant had paid Rs.200 to the regional transport officer for changing the earlier address to address-Surekapuram Colony Jabalpur Road district Mirzapur which is current address of the applicant. It is most unfortunate that at present they have changed my current address to village address on the driving licence which is cause of problem for the applicant. It is most unfortunate that assistant regional transport officer did not follow the submitted form on which it has been categorically mentioned that on the driving licence my current address will be mentioned. But it is most unfortunate concerned staff arbitrarily mentioned my permanent address of the village which is showing arbitrariness in working of the staff. There is no accountability in office.
