This is a humble request of the applicant to the respected sir please take a glance of the following transaction details made by the applicant through the account opened in State Bank of India.

The information sought by the applicant is as follows.

Most respected sir please take a glance of the attached communication of district tuberculosis officer district Mirzapur who made available following information.

When the medicines are not available in the stores of the tuberculosis centre in district Mirzapur then think about the campaign of the government to eliminate the tuberculosis from the country is only on paper and only means to advertise the government on the false and fabricated grounds.

2 In respect of the information sought by you in point no. 2, District Tuberculosis Center, Mirzapur purchased anti-tuberculosis medicine worth Rs. 22007 in the financial year 2021-22.

3 In respect of point no. 3, no anti-tuberculosis medicine was purchased by the Division Hospital Hazrat Imam Yusuf Hospital, Mirzapur in the financial year 2020-21 and anti-tuberculosis medicine worth Rs. 7650 was purchased by the District Tuberculosis Center, Mirzapur.

 Chief medical officer, District-Mirzapur who is also the public information officer in the department Medical and health district Mirzapur may provide the following point-wise information to the information seeker.

1- Right to reason is the indispensable part of sound administrative system so provide the reason of prescribing the digital x ray test of the patients in the tuberculosis isolation centre Mirzapur if such facilities are not available in the  tuberculosis isolation centre Mirzapur and the next test is sputum test is not carried out in the tuberculosis isolation centre by taking the recourse of digital x-ray test which is costly at the private service provider centres and poor patients cannot afford it including applicant.

2-Provide the fund spent in purchasing the medicines in district T.B. hospital and divisional hospital Hazrat Imam Yusuf hospital Mirzapur city in the current financial year i.e. 2022-23.

 3-Provide the fund spent in purchasing the medicines in district T.B. hospital and divisional hospital Hazrat Imam Yusuf hospital Mirzapur city in the current financial year i.e. 2023-24.

4-Provide the Govt order, circular, and office memo which allow prescription of medicines by the doctors working at divisional hospital Hazrat Yusuf Imam Hospital and tuberculosis isolation centre Mirzapur from private medical stores and this practice is currently on its peak.

5- Please provide the reason for non-availability of the entire test facility and medication facility of patients of tuberculosis which is contagious disease, and the government wants to eradicate it quite obvious from its campaign and organisation of a special team for this purpose at the label of central government.

Public information officer, Chief Medical Officer Mirzapur must provide 5 points of information within a stipulated 30 days as prescribed under subsection one of section 7 of the right to information act 2005.
