Subject-The report submitted by the police attached as page 2 and 3 before the U.P.H.R.C. Lucknow is merely a concocted story having no relationship with the truth. Therefore the Honourable commission may order reinvestigation in the matter so that truth may come out and poor Pritesh Kumar belonging to the oppressed section may get justice. 
With due respect, the applicant invites the kind attention of the Honourable Sir to the following submissions as follows.
1-I pray before the Honourable Sir that  51A. Fundamental duties It shall be the duty of every citizen of India (a) to abide by the Constitution and respect its ideals and institutions, the National Flag and the National Anthem;(h) to develop the scientific temper, humanism and the spirit of inquiry and reform;(i) to safeguard public property and to abjure violence;
(j) to strive towards excellence in all spheres of individual and collective activity so that the nation constantly rises to higher levels of endeavour and achievement.

Proceeding याचिका की प्रति लिपि पुलिस अधीक्षक, मीरजापुर को पुनः इस निर्देश के साथ भेज दी जाये कि वह प्रार्थी श्री योगी एम0पी0 सिंह द्वारा दिये गये शिकायती प्रार्थना पत्र में अंकित आरोपों की जांच राजपत्रित अधिकारी से करवा कर आवश्यक वैधानिक कार्यवाही सुनिश्चित करायें तथा कृत कार्यवाही की आख्या आयोग को दिनांक 30.06.2022 तक प्रेषित करें। क्षेत्राधिकारी द्वारा अपनी जांच के दौरान मुख्य चिकित्साधिकारी, मीरजापुर से किसी वरिष्ठ चिकित्सक का सहयोग लेकर तकनीकी परीक्षण करवा कर संयुक्त जांच आख्या आयोग को उपलब्ध करायें। आदेश की प्रति मुख्य चिकित्साधिकारी, मीरजापुर को इस निर्देश के साथ कि पुलिस अधीक्षक, मीरजापुर द्वारा निर्दिष्ट क्षेत्राधिकारी द्वारा की जानी वाली जांच हेतु किसी वरिष्ठ चिकित्सक को संयुक्त जांच हेतु उपलब्ध करायें। पत्रावली दिनांक 31.06.2022 को पुनः पेश हो

3-I pray before the Honourable Sir that according to order a gazetted officer of the police had to carry out investigation with the help of a senior medical expert but it was not followed in letter and spirit quite obvious from the signatures made on the report. Such dealings are made only when facts are concealed and the real matter is put under cover. 
4-It is submitted before the Honourable Sir that  According to the available X-Ray report, delivery was to take place in the month of June 2022 and your three doctors in the report found the patient in labour pain in the beginning of the month of April 2022. 

Please provide the test carried out by the three doctors as mentioned in the report or any other thing on which findings are based. Sir, it is our main concern but neither police is having proper response nor chief medical officer Mirzapur. Whether delivery can be carried out in seven months by the concerned staff of government women hospital is quite obvious from the X-Ray report, the foetus had to mature up to 5 june 2022. Here abortion carried out on the victim cum diseased Rubi by the concerned staff of government women hospital Mirzapur because of corruption.
5-I pray before the Honourable Sir that they have referred for MCH wing for the test but they only managed to get the sign of the Pritesh Kumar and did nothing and credibility of the MCH wing is quite obvious from media report usually published in the dailies and Honourable Sir may take a glance of the last nine pages of the attached PDF document showing the terrible form of corruption in the working of the department of medical and health in district Mirzapur. 
6-It is submitted before the Honourable Sir that the patient had returned to the women-hospital again as no test was carried out by the  MCH wing. Here the most important question is that when they had to do nothing, why had they managed to get the signature of Pritesh Kumar, reflecting their ulterior motive?  
7-I pray before the Honourable Sir that our patient did not get treatment in any other hospital which may be checked from video footage of the aforementioned hospitals. It is most unfortunate that team members put bundles of lies in the reports is a matter of concern.  8-It is submitted before the Honourable Sir that following application submitted by the aggrieved Pritesh Kumar before PMO for justice which may be taken into consideration

An application under Article 51 A of the constitution of India to make enquiry regarding bogus and illogical reports concealing the facts in the matter of cold blooded murder of my wife submitted before the Uttar Pradesh Human Rights Commission by the Mirzapur. Whether it is justified to submit a false concocted story before a constitutional functionary Uttar Pradesh Human Rights Commission, Lucknow.Whether in the name of enquiry report in the state of Uttar Pradesh, criminal activities are covered under the carpet ipso facto. Whether the applicant was not deprived of justice when it is evident from direct and circumstantial evidence  that extreme negligence by the staff of women hospital Mirzapur caused the death of the wife of the applicant. Undoubtedly it is an anarchy which must be curbed. Such unlawful activities by the highly educated public staff has made the government machinery incredible.  Before taking shelter again in U.P.H.R.C. Lucknow, S.P. City, Shrikant Prajapati who is not only an enquiry officer to comply with direction of U.P.H.R.C. Lucknow but also PIO in the office of S.P. Mirzapur may provide the information concerning the report submitted before U.P.H.R.C. Lucknow. 
It is most unfortunate C.M.O. Mirzapur did not provide any of the following five point information to the appellant.  The document attached consists of four pages. First page contains the Ration card of the information seeker cum appellant and report of S.P. City, Shrikant Prajapati addressed S.P. Mirzapur and second, third and fourth page of the attached document is the enquiry report submitted before Uttar Pradesh Human Rights Commission Lucknow under the signature of 1-Dr. U.N. Singh, District Tuberculosis officer, Mirzapur, 2- C.O. City, Prabhat Rai, 3-Dr. Kiran Kala, senior specialist of women diseases and 4-Dr Vivek Singh, Child disease specialist S.N.C.U. Woman hospital Mirzapur. 
State Public Information Officer/ Additional S.P. Mirzapur may provide the following information to the information seeker as follows.
 1-According to the available X-Ray report, delivery was to take place in the month of June 2022 and your three doctors in the report found the patient in labour pain in the beginning of the month of April 2022. 
Please provide the test carried out by the three doctors as mentioned in the report or any other thing on which findings are based. 
2-Please provide the copy of consent of the information seeker in which the information seeker opposed the process of surgery so that the applicant may know when they managed to get a sign of the applicant and how? 
3-Please provide the discharge report made on 16 April 2022 when the patient was first discharged and time and name of staff carried out as well. 
4-Please provide the time when the patient was readmitted and the name of staff who processed it and time span between discharge and readmission.
 5- Please provide the discharge report made on 16 April 2022 finally after the expiration of the patient. After all it is obligatory duty of the caretakers of patients to take discharge reports of the patients, but nothing was provided by the women-hospital Mirzapur.
Please provide aforementioned information to prove transparency in the enquiry carried out by team.
