Grievance Description

The matter concerns the mismanagement in the Hajarat Yusuf Imam divisional hospital Mirzapur. There is no dedication in the staff to serve the people but feel burdened to perform their duties. Unfortunately drawing a huge salary from the public exchequer but has not learnt a lesson from animals like a dog who is known for its loyalty to his master.

Here the matter concerns the early closing and late opening of windows providing slips to the patients. This public-spirited grievance is based on the feedback made available by my younger brother Arun Pratap Singh S/O Late Rajendra Pratap Singh, Village-Nibi Gaharwar, Post-Nibi Gaharwar, District-Mirzapur, Uttar Pradesh, PIN Code-231303. He has applied for the renewal of his driving license in the Regional Transport Office or Road Transport Office Mirzapur and the government of Uttar Pradesh /C.M.O. Mirzapur has nominated Dr Nand Lal for the same purpose to provide a medical certificate as a mandatory part of the cumbersome process of driving licence renewal by currently installed government in the state of Uttar Pradesh.

Arun Pratap Singh reached at 1:30 PM at the window to take the slip from the window but the entire windows were closed. Being a little frustrated made an enquiry on the ground floor to a doctor who told him about Dr Nand Lal. Now he reached Dr Nand Lal and requested him to help him, but Dr Nand Lal asked for a slip. Arun Pratap Singh told him that the entire windows are closed, then he asked to come next and the aggrieved asked him if the next day is Sunday, then he asked to come on the day after tomorrow. Aggrieved Arun Pratap Singh knows that he has already spent petrol of rupees 100 and the day after tomorrow he also faces the same harassment and other expenses, so he requested Dr Nand Lal. Dr Nand Lal asked the aggrieved not to waste time as without a slip he cannot issue the certificate.

Chief medical officers and chief medical surgeons always claim to provide 24 hours service but how can they provide it if the slips will not be issued to patients?

Please provide the name of the staff who sit in the windows issuing slips to patients.

Please provide the opening and closing times of the windows.

I had already made complaints regarding the mismanagement of the concerned hospital which was closed arbitrarily through inconsistent reports. Please help the applicant to overcome anarchy. Form 1 A provided by R.T.O. Mirzapur is also attached to grievance.
