The address of the Geeta Devi is as follows.Geeta Devi W/O Harishankar Maurya Village- Madanpur, Post- Nakati, Development Block- Pahari,  Nakati Mishrauli Pin Code is 231304,Taluk- Mirzapur Sadar District-Mirzapur,  Mobile number-6393679572 Short submissions are as follows.1-Be informed that applicant Geeta Devi W/O Harishankar Maurya made representation before minister of state, health and family welfare, Government of India.2-Consequently, a list of such applicants was forwarded by the office of respected minister as aforementioned on 29/09/2018 to chief development officer, District- Mirzapur  quite obvious from the attached document.3-Detail of the applicant in the aforementioned list is at serial number-10. 4-An enquiry was carried out by the concerned staff of the development block at the behest of the chief development officer District-Mirzapur. 5-No appropriate steps in order to provide the Prime minister Avas was taken officially by the subordinates of the chief development officer Mirzapur.
